Access to HIV services, including prevention, treatment and care, has never been universal, and the conditions for achieving universal access are now deteriorating. Donor withdrawal, shifting aid relationships and pressures on domestic health systems and budgets are eroding gains while structural barriers that have long blocked equitable access, such as stigma, intellectual property rights or procurement bottlenecks, remain firmly in place. New technologies, including long-acting options for prevention and treatment and AI tools, offer a genuine opportunity to transform care, but only if existing inequities are not reproduced in their introduction and scale up. This session will examine what is required to rebuild sustainable, equitable access to HIV services and the roles that governments, donors, civil society and other actors play in achieving it.
